F1 WAIVER REQUEST INFORMATION
As per ALLIANCE policy, Member Hockey Associations cannot issue waivers prior to an actual tryout.
The F1 Waiver request form will open for submission, on the following dates:
U10, U11, U12 and U13
- request form will open at 7:00am on Saturday, April 20, 2024
U14 & U16
- request form will open at 7:00am on Monday, April 22, 2024
U15
- request form will open at 7:00am on Tuesday, April 23, 2024
After the form is completed, it is sent directly to the WMHA Board Member authorized to issue the F1 Waiver.
The Waiver will be emailed to the address you have provided.
Please allow, at minimum 4 hours to receive the F1 Waiver as we work through the request process.
